We are currently seeking an experienced Home Care Coordinator to join our team. Role Details

  1. Position: Home Care Coordinator
  2. Location: Lower North (onsite)
  3. Hours: Part-time, 0.4FTE, working days to be negotiated
  4. Reporting to: Director of Care
  5. Remuneration: Registered Nurse - Nurses Award Equivalent Aged Care Level 2 (first 3 years) $51 per hour + Super + Salary Packaging
  6. Travel Requirement: Covering Lower North region.

Key Responsibilities

  1. Conduct client care assessments and oversee ongoing care planning
  2. Maintain client compliance and manage a caseload of clients
  3. Coordinate and deliver personalised care aligned with client needs, care plans, and allocated budgets
  4. Perform general administrative tasks related to the role.

Skills and Experience

  1. Relevant qualifications (Registered Nursing highly regarded)
  2. Experience in aged care or mental health case management (preferred)
  3. Knowledge of My Aged Care, Home Care Packages, and NDIS frameworks
  4. Proven experience in assessing and planning community-based care
  5. Proficiency in Microsoft Office 365, SharePoint, Outlook, and rostering software
  6. Excellent verbal and written communication skills with strong customer service abilities
  7. Ability to handle incoming calls professionally and courteously
  8. Strong organisational, problem-solving, and decision-making skills
  9. Collaborative team player with a proactive attitude
  10. Ability to build strong relationships with external organisations for client referrals
  11. Commitment to confidentiality and privacy in line with company policies.
